None. No assertion has ever been flagged by enough independent operators, holding enough earned reputation, inside the window. That is the outcome everybody wants and it is worth stating plainly rather than leaving as an empty table.
Near misses
Assertions that some nodes flagged without reaching the bar, and exactly which condition stopped them. These are published so the threshold is legible as a real mechanism rather than a number in a document.

No node in this network can dispute a state root — the allowlist is closed and we are not on it, which we demonstrate rather than assert. What an incident unlocks is the Citizen Challenger Fund: Verderer-rank operators may vote, weighted by earned reputation, to pay one of the allowlisted actors to file the challenge on the network’s behalf, with the aggregated and checkpointed attestation log as the evidence.
Only the senior rank votes, and reaching it takes five thousand confirmed attestations — months of being right, which cannot be bought or hurried. Money is being spent on an irreversible external action; the people deciding should be the ones with the longest record of correct work.
